الأستاذ الدكتور محمد عصام فكري هو رئيس قسم أمراض باطنة الأسبق بجامعة الاسكندرية. وهو مؤسس وحدة طب المسنين (كلية طب جامعة الإسكندرية). وهو أحد الرعيل الأول لمؤسسي طب المسنين في مصر.

The upper end lies opposite the sixth cervical vertebra in the neck and the lower end lies opposite the sternal angle anteriorly and the space between the fourth and fifth thorcic vertebra posteriorly. The trachea divides into the right and left main bronchi, the left running somewhat more horizontally than the right. The right main bronchus gives rise to three lobar bronchi, the left to two. Each lobar bronchus divides in turn into segmental divisions. The trachea, right and left main bronchi and both lower lobe bronchi are outside the lung subs tance. All other bronchi including the upper lobe bronchi are situated within the lung. Faculty of Medicine. General Medicine department. ملخص: The deep reflexes are elicited by striking a tendon and so stretching it. This forms the sensory inpulse which passes up afferent nerve fibres to the spinal cord. Internuncial fibres convey the impulse to the anterior horn cells, which discharge a motor impulse to the muscles supplied from that segment. Diminution or absence will result if the reflex arc is interrupted by injury or disease. It may be interrupted in the’motor limb, in the sensory limb or in the spinal cord itself. Peripheral neuropathy and root lesions are the commonest causes for absent tendon reflexes. Tendon reflexes are also abolished by spinal cord shock., e.g. severe injury to the cord. Lesions of the muscles themselves (dystrophies, involvement in scar tissue) will, of course, prevent the muscular contraction which is the visible evidence of the reflex. Loss of tendon reflexes is bilateral in states of coma. Lastly it must be borne in mind that in some persons the deep reflexes are sluggish and occasionally absent without pathological cause, especially the ankle jerks in the elderly. Abolition of a reflex by a segmental lesion of the cord is particularly valuable in localization. Exaggeration of the deep reflexes is an important sign of an upper motor neuron lesion, especially if unilateral or accompanied by other signs. They were studied for the prevalence of I3H . The patients with ISH were divided into two groups: 1.- Group I 41-60 years (36 Cases) 2. Group II 61-80 years (2? Cases) All subjects with ISH were suspected to: A- Complete history taking. B- Full clinical examination, C- Laboratory investigations including, fasting blood sugar, blood Urea, serum cholesterol, Complete blood picture, Complete Urine analysis, EGG, and plain X-ray chest. Special investigations, if needed as T~ and T. levels. The prevalence of systolic /diastolic hypertension was 28.5/5 , it has been found in 20,5?° of males and in 30.5$ of females. Isolated systolic hypertension wes found in 63 patients .1$)i it has been found in 13 meles and 50 females. They were chosen from outpatient clinics of the Alexandria Uni varsity Hospital. All subjects were free from any present or previous hepatic disee3e , also they were free from any disease that would affect liver size. During ultrasonography of the liver a transverse and a sagittal liver dimensions were measured, There wag no significant change between both groups as regards transaminaaes and sorud billrubin. A detailed history, using a defined data for every patient followed by endoscopic examination using only a local pharyngeal an aesthetic. The patients were divided in to 2 main groups: 1- The study group: which was composed of 30 elderly patients complaining of dyspepsia. 2- The control group: of 30 patients subdivided into three subgroups: a-Composed of 10 elderly patients who were not dyspeptic . b-Composed of 10 young patients who were dyspeptic. c-Composed of 10 young patients who were not dyspeptic. Department Of Neuropsychiatry. ملخص: five elderly subjects and 20 depressed young adults •with no present or past history of any severe somatic disorder were subjected to standard psychiatric and I physical examination, social investigation 33 well aa the Arabic Version of Beck’s Depression Inventory (L.I1.1.) and the Multidimensional Observation Scale for Elderly Subjects (M.O.S.E.S.). I!he aim of this work is to study and identify the presence of depression and demential behaviour as well as to assess the presence and degree of’ their combined presenta¬tion among the aged. Medical examination at that time is considered particularly important. Staff working in contact with the elderly should have a common body of gerontoiogical knowledge besides proper training. This is essential to enable them to serve adequately the elderly* This study was conducted firstly; to assess the knowledge of the personnel working in contact with the elderly in geriatric homes, as regards different aspects concerning the elderly, and secondly} to assess the medical examination done to the residents on admission to the homes. For this purpose, a questionnaire form was prepared to assess personnel’s knowledge through .their interview, and a check-list was constructed to assess the admission medical examination through records’ review. The study was carried out in the twelve geriatric homes in Alexandria (five Egyptian and seven non-Egyptian). إجتمع مجلس إدارة الجمعية رئاسة د. عبده محود سلام وبحضور أعضاء مجلس الإدارة: د. محمد صادق صبور ود. يحيى مهران ود. محمد صادق فودة ود. عبد المنعم عاشور ود. محمد طلعت عز الدين ود. مسعد محمود سلام ود.زكريا مهران. إعتذر عن الحضور د. محمد عصام فكري ود. محمد الظواهري لظروف طارئة ود. محمد البانوبي لسفره خارج الجمهورية.

وبدأ المجلس أعماله على النحو التالي:

أولا: قام أد/عاشور سكرتير الجمعية بتلاوة محضر الجلسة السابقة بتاريخ 16-11-1988 وتمت الموافقة على ما جاء بها.

ثانياً عبر إستعرض المجلس طلبات الترشيح لمجلس الإدارة ...........

ثالثاً: إستعرض المجلس طلبات العضوية الجديدة المقدمة للجمعية ..........

رابعا: تشكيل لجان لمتابعة تنفيذ توصيات المؤتمر الأول للجمعية المصرية لطب المسنين (1989) المنعقد بتاريخ 15-1-1989.

1. 1) بالنسبة للتوصية الأولى تشكل لجنة من السادة (د.يحيى مهران ود. محمد صادق صبور ود. سعد سلام]]) للعمل على تنفيذ ما جاء بها.
2. 2) بالنسبة للتوصية الثانية فوض المجلس د. عبده محمود سلام رئيس مجلس الإدارة ومن يراه لتنفيذ ما جاء بها.
3. 3) بالنسبة للتوصية الثالثة قرر المجلس التحرير إلى الجمعية المصرية لصحة المسنين لتنفيذ ما جاء بها وفوض المجلس د. عبد سلام ود. عبد المنعم عاشور لتنفيذ ذلك.
4. 4) بالنسبة للتوصية الرابعة قرر المجلس التحرير للنوادي الرياضية ورعاية الشباب على ان يتولى ذلك د. يحيى مهران.
5. 5) بالنسبة للتوصية الخامسة قرر المجلس أن يحرر بذلك إلى رابطة قدامى الموظفين وأرباب المعاشات بشارع عماد الدين على أن يتولى تنفيذ ذلك د. محمد طلعت عز الدين.
6. 6) بالنسبة للتوصية السادسة قرر المجلس التحرير للجمعية المصرية لصحة المسنين (الأستاذ يحيى درويش) والجمعية العامة لرعاية المسنين ووزارة الشئون للعمل على تنفيذ ذلك ويشرف على تنفيذ ذلك د. عبده محمود سلام ود. عبد المنعم عاشور.
7. 7) بالنسبة للتوصية السابعة قرر المجلس التحرير تفويض د. عبده محمود سلام ود. هنية محمد علي عبد الله ود. عبد المنعم عاشور للعمل على تنفيذ ذلك والكتابة لشركات الدواء.
8. 8) بالنسبة للتوصية الثامنة قرر المجلس تفويض د. عبده محمود سلام للعمل على تنفيذ ما جاء بهذه التوصية.
9. 9) بالنسبة للتوصية التاسعة تقرر التحرير للجمعية العامة لرعاية المسنين والجمعية المصرية لصحة المسنين ووزارة الشئون للعمل على تنفيذ ما جاء بهذه التوصية وتفويض د. عبده محمود سلام ود. عبد المنعم عاشور لتنفيذ ذلك.
10. 10) بالنسبة للتوصية العاشرة قرر المجلس التحرير لأجهزة الإعلام وجمعيات رعاية المسنين للعمل على تنفيذ ما جاء بهذه التوصية.

خامساً: وافق المجلس على إصدار نشرة إخبارية للجمعية كل ثلاثة شهور ويشرف عليها د. عبد المنعم عاشور ود. محمد طلعت عز الدين]] ود. زكريا مهران.

سادساً:وافق المجلس على ما جاء بالمذكرة المقدمة من د. عبد المنعم عاشور ؟؟؟؟ برنامج للندوة العلمية حول موضوع (نظم الخدمة الطبية للمسنين في مصر والولايات المتحدة ومقارنة إقتراحات بتحسين نظم الخدمة الطبية للمسنين في الولايات المتحدة ومصر) على أن يقوم مشروع الفولبرايت بتنظيم وإستضافة المشتركين في هذه الندوة على أن يحضره جميع أعضاء مجلس إدارة الجمعية ومن يرى الدكتور عبد المنعم عاشور دعوتهم للحضور من المهتمين بشئون المسنين وذلك يوم 23-2-1989.

وإنتهى المجلس من أعماله في تمام الثالثة بعد الظهر.

ثانياً عبر المجلس عن أسفه الشديد لوفاة مؤسس الجمعية ورئيس مجلس الإدارة للفترات السابقة المرحوم الأستاذ الدكتور/ عبده محمود سلام. وقرر المجلس إشهار؟ إسم المرحوم/ رائدا لطب المسنين في مصر وأول رئيس للجمعية.

ثالثاً: إنتخب أد/ محمد صادق صبور رئيساً لمجلس الإدارة في الدورة الحالية خلفاً للمرحوم الدكتور/ عبده محمود سلام.

رابعا: تم عمل القرعة وإسقاط عضوية ثلث أعضاء مجلس الإدارة وأسقطت عضوية السادة الأساتذة الدكاترة: زكريا مهران ومحمد صادق صبور ومحمد صادق فودة. ........

ثامناً: أشار أد/ محمد الظواهري إلى أهمية زيادة عضوية الجمعية بالطرق المختلفة مثل الإعلان بالجرائد عن موعد الجمعية العمومية. تحرير خطاب لرؤساء الشعب المختلفة للتعريف بشعبة المسنين وترشح من يرغب في عضوية طب المسنين.

تاسعاً: وعد د. محمد صبور بالبحث عن موارد مالية للجمعية.

عاشراً: يفوض مجلس الإدارة أد/ محمد صادق صبور في متابعة موضوع إنشاء قسم لطب المسنين بكلية طب عين شمس.

إحدى عشر: يفوض مجلس الإدارة أد/ محمد عصام فكري في متابعة موضوع إنشاء قسم لطب المسنين بكلية طب الإسكندرية.

إثنى عشر: ناقش المجلس موضوع زمالة طب المسنين المقدمة من أد/ عصام فكري وحيث أن المبدأ هو أن الزمالة تكون حيث لا يوجد شهادة جامعية. وحيث يوجد ماجستير (تصحيح للفظ ما) لطب المسنين بعين شمس فيؤجل موضوع الزمالة حالياً.

ثالث عشر: يسجل المجلس نجاح التعاون بين وحدة طب المسنين بطب عين شمس مع هيئة التأمين الصحي ويبارك التعاون مع وزارة الصحة في مجال تأهيل فرق إجتماعية طبية متكاملة في التخصص وإنشاء خدمات للمسنين في التأمين الصحي بوزارة الصحة.

رابع عشر: تحدث الكتور/ محمد الظواهري عن المجلة الطبية المصرية ورحب بنشر أي بحوث خاصة بالمسنين بالمجلة.

خامس عشر: نظراً لتخصيص يوم الإثنين ................... لطب المسنين في الندوات الطبية التنشيطية
