You are on the hospital inpatient service and are taking care of Patty Smith, a 70 year-old woman admitted three weeks ago with shortness of breath and lower extremity edema. This is her third admission to the hospital in the past six months. She has a history of congestive heart failure (CHF), hypertension, coronary artery disease and osteoarthritis. She lives at home with her husband.

She is now medically stable and your team is getting ready to discharge her.
