Improve care for frail seniors by developing a Network of Centres of Excellence in Practice Based Interprofessional Education and Interorganizational Collaboration in Geriatric Care, supporting the initiatives outlined in HealthForceOntario’s Interprofessional Care: A Blueprint for Action
Increase the competency of primary health care professionals in three core areas: clinical geriatrics, interprofessional care and interorganizational collaboration (GiiC). The strategies used to reach these objectives were to:
Create a GiiC knowledge-to-practice network with hubs in the Regional Geriatric Programs (RGPs) and in the Centre for Education and Research on Aging and Health at Lakehead University in Thunder Bay, Ontario
Consolidate a team of GiiC Resource Consultants in identified geographical regions to provide training and mentoring to GiiC Facilitators
Develop a pool of GiiC Facilitators situated in Family Health Teams and Community Health Centres to assist their teams in the care of frail elderly
Create GiiC teaching resources and facilitation tools with an on-line repository
Implement a sustainability plan for each network hub consistent with each group’s needs, resources and capabilities
